Becoming a patient
At Headaches and Pain Care Center, you are the most important member of the treatment team. Your experience and observations of headaches and other pain contribute to your care and provide our clinicians with information for your personalized treatment plan. Our responsibility to you is our dedication to establishing a innovative health care model that enhances your life and your well-being. We want you to understand the importance of taking an active role in the treatment of your headache.
Your care begins with your first phone call to our facility. When you call, or when your referring doctor calls, the friendly receptionist will get the necessary registration information. You will be asked a few questions regarding your reason to visit our office.
Once scheduled, you will be contacted by phone a day prior to your arrival time and date. It will also contain papers that you will need to fill out and bring with you on the day of your appointment. We would like to advise you to come at least 15 minutes ahead of your schedule time to allow enough time to fill out patient information papers.
Once the clinician has examined you and assessed your situation, your treatment plan will be developed with your assistance. The clinician may request that your treatment go beyond just a physical examination. You may be required to follow-up with other medical tests, Some lifestyle management or schedule a consultation with a physical therapist. Whatever the recommendation is, it is made by Dr Patel with the belief that it will make a difference for you.
Overall, our goal is to provide you with a comprehensive, inclusive and integrated approach to life without headaches or pain or with minimum sufering from these pains. And to succeed, we need you as an active paticipant in your treatment. We need you to follow up with your appointments and to comply with medication and lifestyle recommendations. We are only successful as a team if it includes you.
Insurance
Headaches and pain Care Center is an independent facility and out of network with many insurance companies. However, we do participate with numerous insurance companies, including Blue Cross/Blue Shield, Anthem, Indiana Carpanters, Tricare/Triwest, Medicare. Our billing department will verify insurance benefits and inform you if your benefits are out of network. Insurance claims are filed as a courtesy for our patients. Some procedures performed at our center are not covered by insurance, and charges for those procedures are the responsibility of the patient.
Payment in full is expected at the time of service, including co-pay and co-insurance amounts.
Prior Authorizations: If your insurance requires prior authorization to see a provider at headaches and pain care center, it is the patients responsibility to obtain that authorization prior to the first appointment.
